Diagnostic mammograms can include a variety of breast imaging views to address an area of concern that may have been found during an initial screening. They can also be helpful when evaluating individuals with a family history of breast cancer or those experiencing symptoms such as changes in the breast skin, nipple discharge, or a concerning lump.

What you need to know

What can I expect if I undergo a diagnostic mammogram?
During a diagnostic mammogram, here's what you can generally expect:
  • Preparation: Upon arriving, you'll change into a hospital gown and should avoid wearing deodorants or perfumes as they can interfere with the images.
  • Positioning: The diagnostic mammogram procedure is like a screening mammogram. You'll stand by the mammography machine, where a technologist will position your breast on a platform and gently compress it with another plate from above for a few seconds. The positioning may differ from what you may have experienced during a screening mammogram, however the imaging process is the same. This may be repeated if more than one view or image is needed. You may be asked to wait while a doctor reviews the image to determine if more pictures are needed; if that happens know it may simply be that the doctor needs a different angle or view of the area in question to complete his or her assessment.
  • Imaging: The actual imaging takes just a few minutes. You might need to change positions to capture different views.
  • Compression: Compression is brief and necessary for clear images, though it might cause some discomfort.
  • Duration: The entire procedure typically takes about 20-45 minutes.
  • After the scan: You can immediately resume normal activities.

Yes, a doctor's order is required, even if it is a call back from a screening mammogram. If you are called back, we will assist you in obtaining your primary care doctor's order.
What is the difference between a screening and a diagnostic mammogram
A diagnostic mammogram is more detailed than a screening mammogram. Screening mammograms are routinely performed to detect breast cancer in women who have no concerning symptoms. On the other hand, diagnostics mammograms are needed when concerns are raised after screening results. The result on the screening prompts your physician to further test the alarming breast tissue.
Why do I have a bill for a diagnostic mammogram if it is the result of a call back from a screening mammogram?
Due to a diagnostic mammogram being used to make a diagnosis rather than a preventative and regular screening, some insurances may not cover the full cost like they would a screening mammogram. In these cases, patients most likely would get billed a copay, depending on what health insurance they may have.

Diagnostic Breast Ultrasounds

Diagnostic ultrasounds are oftentimes ordered by physicians in combination with a diagnostic mammogram. This technology uses sound waves to evaluate worrisome areas that can be felt, such as a breast lump or any other point of concern found in a mammogram. This technology is better suited to evaluate a specific area while a mammogram evaluates the entire breast.

What you need to know

What can I expect if I undergo a diagnostic ultrasound?
During a diagnostic ultrasound, here's what you can generally expect:
  • Preparation: Upon arriving, you'll change into a hospital gown and should avoid wearing deodorants, perfumes, or lotions. You should also wear clothing that you can easily remove.
  • Positioning: The diagnostic ultrasound requires you to lie on your back on the exam table. The technician will ask you to rest your arm over your head. They might also ask that you lie on your side with your arm resting over your head or sit straight up with your arm over your head. These different positions ensure that the technicians evaluate all angles of the breast tissue. The ultrasound technician will use a clear, water-based gel on your breast and navigate their way through your breast tissue using a transducer. A transducer emits sound waves in small bundles or pulses that are reflected off the breast tissue back to the transducer to get images of the inside of the breast.
  • Imaging: The actual imaging takes just a few minutes. You might need to change positions to capture different views.
  • Compression: Compression is not a part of this exam; the technician may apply slight pressure to ensure the transducer is picking up images, but it is not like compressions you would encounter in a mammogram.
  • Duration: The entire procedure typically takes about 15-30 minutes.
  • After the scan: You can immediately resume normal activities.

Breast MRIs can be used in combination with screening mammograms. They add an extra layer of prevention when screening individuals who may be at a higher risk of developing breast cancer. They are often used in the early stages of detection and can provide doctors with more information about a breast disease diagnosis.

What you need to know

What can I expect if I undergo a breast MRI?
During a breast MRI, here's what you can generally expect:
  • Preparation: Upon arriving, you'll change into a hospital gown and should avoid wearing anything with metals (jewelry, hairpins, watches, etc.).
  • Positioning: The MRI machine has a large, central opening. The technician will have you lie face down on a padded table that slides into the machine, where your breasts will fit into a hollow space or indent in the table. The hollow space has coils that receive signals from the MRI machine. You may hear loud tapping and thumping noises during this exam, but you won't feel anything. The machine creates a magnetic field around you that sends radio waves to your body. Please let your technician know if you are uncomfortable with being in small spaces, they will be watching you from a different room, but you can speak to them through a microphone.
  • Imaging: The imaging will be reviewed by a radiologist and results are provided to your ordering physician and available in our patient portal, usually in a day or two.
  • Compression: Although there is no compression during a breast MRI, you do lay on your stomach for the study.
  • Duration: The entire procedure typically takes about 30 minutes to an hour.
  • After the scan: You can immediately resume normal activities.

Breast Biopsies

Breast biopsies can be minimally invasive procedures that are used to further evaluate breast tissue. The sample of breast tissue will be removed and then sent to a lab where lab technicians will determine if the tissue is benign/noncancerous or cancerous. The results of your biopsy will allow your doctor to determine if further treatment or surgery is needed. There are several types of breast biopsies: fine-needle aspiration, core needle, stereotactic, ultrasound-guided core needle, and MRI-guided core needle biopsies.

What can I expect if I undergo a breast biopsy?
During a breast biopsy here's what you can generally expect:
  • Preparation: Upon arriving, you'll change into a hospital gown and should avoid wearing deodorants or perfumes. Additionally, you should wear/bring a tight-fitting bra like a sports bra.
  • Procedure Options and Positioning: During the procedure, a core needle biopsy is taken to assess the area of concern on a mammogram or ultrasound. A thin hollow needle will be used to remove tissue samples from the lump. Several samples may be collected and analyzed. There are different methods used for biopsies.
    • Stereotactic biopsies are used for suspicious or indeterminate calcifications or an area of distortion found on previous imaging. The doctor uses a mammogram to pinpoint the location of the concerning lump. You will lie down on a padded table with one of your breasts positioned in a hole in the table. In some instances, you may be in a seated position, in this case, you may need to remain in that seated position for 30 to 60 minutes.
    • Ultrasound-guided core needle biopsies are often used when there is a suspicious palpable lump. During this biopsy, you lie on your back or side on an ultrasound table. The technician will hold the ultrasound wand against the breast as they locate the mass, make a small incision to insert the needle, and take samples of tissue.
    • MRI-guided core needle biopsies use the guidance of an MRI machine to capture cross-sectional images of the breast and combine them using a computer to generate 3D pictures. You will lie face down on a padded scanning table where your breasts fit into a hollow depression on the table. The images provide the doctors with guidance on where the exact location of the lump is. Once located, a small incision is made to allow the needle to be inserted and tissue samples will be collected.
  • Duration: The entire procedure typically takes about 60-90 minutes.
  • After the biopsy: Pressure and a bandage will be applied to the incision site. Medication for discomfort such as Tylenol, avoid anti-inflammatory drugs. The radiologist will give you post-biopsy instructions, results should be provided to you within 3-5 days.
