What to Expect During a 3D Mammogram: A First-Timer’s Guide

If you’re heading in for your first 3D mammogram, it’s completely normal to feel a little uncertain. You’ve probably heard the word “mammogram” tossed around during checkups, or maybe it’s been on your to-do list since you turned 40. But what happens during the appointment? And how does a 3D mammogram differ from the traditional kind?

Let’s walk through the process so you know exactly what to expect during a 3D mammogram—and can head into your visit with confidence.

First Things First: Why 3D Mammogram?

A 3D mammogram (also called digital breast tomosynthesis) uses X-rays to take multiple images of the breast from different angles. Those images are then layered to create a more detailed view of the breast tissue. This makes it easier for radiologists to spot abnormalities—especially in people with dense breast tissue—and can reduce the chance of being called back for additional images.

What does it cost?

If you’re age 40 or older, annual screening mammograms are covered by most health plans with no out of pocket costs. If you’re at high risk, you may also be covered before age 40. Any necessarily follow up imaging will typically be covered similar to other doctor’s office services.

Before Your Appointment

There’s not much prep needed. You’ll want to avoid wearing deodorant, lotion, or powder on your underarms or chest on the day of your exam—these can appear on the images and interfere with the results. It’s also a good idea to wear a two-piece outfit since you’ll be asked to undress from the waist up.

Try to schedule your appointment for a time when your breasts aren’t feeling tender or swollen, which is often the week after your period. This can help make the experience more comfortable.

What Happens During the Exam

Once you’re in the exam room, a technologist will guide you through the process. They’ll position one breast at a time on a flat surface and gently compress it with a paddle. This compression may feel a little awkward or uncomfortable, but it only lasts a few seconds. It helps spread out the tissue for clearer images and reduces the radiation dose.

Unlike traditional 2D mammography, the 3D machine takes multiple images in an arc, like a mini CT scan. You’ll typically stand for the exam, and the technologist will take pictures from different angles. The whole thing usually takes about 20 minutes.

One of the most common concerns with mammograms is discomfort during compression. That’s where the comfort paddle comes in. After the Exam

There’s no downtime—you can go about your day right after. A report of your results will usually be sent to your doctor the same day.
